Once you’d made an appointment, please download, print, and fill out the intake forms before the initial session. Bring these two forms with you to your first appointment. This helps us both to maximize our time together.
Counseling services at Covenant Presbyterian Church are provided at no cost to church members. In the event a referral is needed to Journey Counseling Ministries due to a full schedule or a desire for a same gender counselor, Covenant has generously setup a membership assistance fund to help subsidize this fee-based counseling. Please discuss this with me during your first appointment.
During the counseling process, it may become important to contact one or more outside parties, such as family members, spouses, general medical doctors, psychologists, psychiatrists, and pastors. If this becomes necessary, you will have the opportunity to talk with your counselor about the Release of Information form and fill one out in session.
